CAREPATH Project

CAREPATH Project (Collaborative Action and Research for Engagement, Persistence and Adherence in Treatment & Health) is an international research and implementation project funded by the Innovative Health Initiative (IHI), running from May 2025 to April 2030. Its goal is to improve treatment adherence and persistence in patients with chronic diseases—specifically obesity, type 2 diabetes, and cardiovascular diseases.

The project brings together 42 partners from Europe and the USA, representing a broad spectrum of institutions—including universities and research centers, healthcare providers, as well as pharmaceutical and technology companies. Within the project, a comprehensive CAREPATH Toolbox will be developed, integrating proven, effective digital and non-digital interventions to support long-term treatment, tailored to local healthcare system contexts.

These tools will be tested in diverse clinical settings, using real-world data. Pilot studies will be conducted in six countries with different healthcare systems: Germany, Israel, the Netherlands, Poland, Spain, and Sweden.

CAREPATH focuses in particular on adherence to modern therapies, such as GLP-1 receptor agonists, SGLT2 inhibitors, and lipid-lowering medications, examining their effectiveness, side effects, administration methods, and the risk of premature treatment discontinuation. A key objective is to enhance communication and data sharing between patients, healthcare providers, and other stakeholders, ultimately improving treatment outcomes, patient quality of life, and the efficiency of the healthcare system.

The Medical University of Lodz, through the Medication Adherence Research Centre at the Department of Family Medicine, is leading Pilot No. 1, conducted in the Polish primary care setting. The study will focus on improving adherence in the treatment of cardiovascular diseases.
